If you're interested in Long-Term Services and Supports, contact RMOLTC at 719-457-0660 ext. 100 and ask to speak to a Resource Navigator.
The Resource Navigator will complete a phone screening.
If an assessment is needed for a Health First Colorado (Colorado's Medicaid Program) Long-Term Care program, the Resource Navigator will confirm if applicant is currently receiving Health First Colorado benefits.
If applicant is receiving benefits, a referral will be sent to the local department of human services.
If applicant is not currently receiving Health First Colorado (Colorado's Medicaid Program) benefits, the application will need to be completed with your local department of human services or at a Health First Colorado (Colorado's Medicaid Program) application site.
You can also start the process with your local department of human services or through the Colorado PEAK Portal, and a referral will be forwarded to RMOLTC.
